In order to receive CE credits for the sessions you attend, the form must be stamped by the session monitor and you must register your name and address at the IAFP Booth. Before leaving the conference, please turn the form in to the IAFP booth during exhibit hours. After exhibit hours, please return this form to the EMS Continuing Education Box in the main lobby near the registration desk. After the conference, you may also mail this form to the IAFP office.
